I always start with sheer inspiration, then apply structure and mechanics. Return to inspiration, mechanics and so on.
I am feeling so much better about my own progress now that I've been home for 3 weeks with all my time available for writing. And I do spend ALL of my time writing.
Secondly, I do find that outlines are best for me, though, like you say, I am open to adding surprises that the characters or muse might have in mind for me.
