I'd say watch it tonight and tomorrow, but if it's not cleared up come Monday, maybe have Sam's ped look at it.
When my daughter was about 1 and 1/2 every time she ate strawberry yogurt she would break out in little red bumps. Now she is four and eats strawberries all the time and never has that problem anymore.
Poison Ivy?

Heat rash is what it looks like, but I would hardly think that based on where you live, unless your house is hot.
Perhaps something he ate. Anything new?
How about new clothing? Certain materials can cause reactions. It seems to be more red where the tag might lay on his back.
Does he have similar rash on his stomach? if he does, it might be roseola rash. Check with the doctor.
If it got bigger with the warm water...probably the hives
