I had to fill one out for the first time in 8 years. If your getting Social Security or Social Security Disability you have to file to get the rebate. Ridiculous. Fill out a stupid form with name, SS#, address and amount of social security disability and everything esle is 0000000000000 ad nauseum.

Sign, seal and deliver. Did it today.
