What is it with Dentists?
About 2 years ago, I had root canal therapy in one of my molars. My own dentist of 23 years couldn't complete it and referred me to an Endodontist. The Endodontist completed the procedure, put a temporary filling on top of it and hopped a plane to Hawaii for his vacation.
There was an underlying infection that wasn't apparent at the time, which erupted with the closing of the tooth with the temporary filling. Enter excruciating pain. Dr. Hawaii was obviously not someone I could go to so I called my Dentist and told the nurse I needed an antibiotic and something for pain. She called me back and told me that Dr. had called in a prescription for an antibiotic but wouldn't give me anything for pain. I was furious! This wasn't the first time he had done this. A family member (who also sees this Dentist) brought me some of their pain medication and it saw me through until the antibiotics kicked in. I fired that Dentist and I told him that if he isn't going to care about the pain management of his patients, I wouldn't be one of them.
Fast forward. A couple of months ago I went looking for a new Dentist. In the initial consultation with this one, I told that story and stressed to him that I couldn't be a patient of someone who didn't care about the pain that is inevitable with certain dental issues. He told me that he understood completely and it wouldn't be a problem.
I have a molar that was cracked and a couple of weeks ago it cracked further, exposing most of the tooth. This week, infection set in and included my ear and the glands in my neck. The pain was unbearable. I got through the night with 1/2 of a Percocet (Ocycodone) that a family member had given me. Nothing else had worked.
The next day I spoke with this Dentist over the phone. It was decided that the tooth couldn't be repaired and needed to be extracted and he referred me to an oral surgeon. The infection needed to be dealt with and he was prescribing an antibiotic. I told him that the Percocet had worked the night before and nothing else had and that is what I wanted.
The pharmacy called me about the prescription and I asked what Dentist had prescribed. "Lortab." AKA-Vicodin. I told the pharmacist to not bother filling that prescription because I've taken Vicodin before and all it does is make me dizzy and nauseous and does nothing for pain. I then called the Dentist. He said he didn't want to prescribe the Percocet because he didn't like that drug. I told him that HE didn't have to like it, he wasn't taking it. He told me that he wanted me to "try" taking 4 Advil and 2 Tylenol at the same time and see if that works and that he'd prescribe an anti-inflammatory for me because he had been at a conference recently and someone told him this particular drug worked. I told him that with the pain I was in, I didn't wish to be an experiment, I wanted the pain to STOP! "If" his attempts didn't work, it was ME who would continue to suffer.
He said he didn't like Percocet because it was a heavy narcotic and he didn't want me to become addicted. I assured him that I wouldn't become addicted in 4 days. He then told me that he wasn't really that familiar with Percocet. He's totally against something he's not familiar with? I explained to him that I've suffered from migraine headaches for over 30 years and I've taken most every narcotic known and I would actually have permission to be addicted! The fact that I've never become addicted, don't even like narcotics and take them only when absolutely necessary, ought to tell him something! The prescription is closed ended-no refills. This drug worked to get rid of the terrible pain I was in, so, what is the problem giving it to me? He refused. I told him that I'd be seeking other services for Dental work. I'm sure if I were an addict and had used up the medical community and was now in the dental tier seeking drugs, my name would be going around the community because of it.
I then called my personal Dr. who I've seen for 25 years and told his nurse what the situation is. He wrote the prescription for Percocet for me and I went to pick it up an hour later. It was an awful night and the following day until the antibiotics kicked in, but at least I could keep the pain at bay with something that worked. I have 6 of them left and I'm now hoarding them for oral surgery next week so I don't have to go through this all over again.
What is it with Dentists and their inability to manage pain? Surely, they must understand how certain dental problems or procedures will be extremely painful, but they'll tell you to "take Tylenol" or give you some lame pain medication that will not touch the pain, just to say they wrote a prescription! Why would they want their patients in such pain when they could easily do something about it? And they wonder why so many people don't see their dentist often enough? People tend to gravitate away from traumatic experiences.

And Nanci, what a PAIN to have to go through all those steps to get some pain med. I can understand trying to get to the basis of the problem but how about some pain medication on the way.
Nanci- OMG- this is awful. I can't imagine having to go through all of that just to get some relief! To me, this is a violation of the oath they took to "do no harm." Leaving people in pain, in my opinion, is just that- doing harm.
Jadan- thank you. Yeah, I couldn't understand the idiocy of his decision. I think he's "heard" bad things about the drug and instead of being a responsible professional and reading up on it and dealing with HIS patient, he was dealing with his own prejudice. He won't be seeing me again.
Linda- I'm with you. If the drugs are made for specific uses and you have a patient that falls into that category- prescribe the medication! You are right- it's torture and it's just not acceptable. But they have the freedom to do it and there isn't much a person can do. I feel for Nanci. I can't imagine going through that and I'd go medieval on them in a heartbeat.
I shouldn't have to bother my physician because another medical "professional" feels uncomfortable and can't seem to move off that mark. His unwillingness to act put both me and my physician in a position we should never be in. If my physician wanted to be a Dentist, I'm sure he would have gone to school for that instead of Internal Medicine.
Sometimes the decisions one has to make aren't easy. But to unload the decision making onto another professional because he's "uncomfortable" is lame. Maybe part of the reason you don't see Dentists prescribing this kind of medication to their patients is that they are all abdicating the responsibility to other medical professions for the care of THEIR patients pain.
No, I won't be giving him another chance. I was in extreme pain and he knew that and he was completely comfortable leaving me in that kind of pain. What if I didn't have a physician to go to? How many others are out there in the same position who weren't as lucky as I was? I can't imagine anyone having to deal with that out of the "discomfort" of a medical professional to prescribe a medication that works to provide pain relief. I won't subject myself to that lack of care again.
I ended up calling the emergency line of my OBGYN at 9 PM when the shots totally wore off and my jaw felt like she broke it prying the tooth out. He wrote me a scrip for vicodin, and said it was safe to take during certain weeks of pregnancy, and told me when I HAD to not be taking them. I felt like I had been punched in the face by a pro boxer. The vicodin did not kill the pain, but took the edge off enough for two days while I healed.
I have not been back to the dentist since. I still need 3 molars + wisdom teeth pulled, which I had planned to have done right after baby was born. She is almost 6 months old now.
I can not believe how bad it hurt to have them "lever" the tooth out, there was no pulling there. It was all done with that bar they use for leverage. I seriously think it caused some damage to my jaw. It pops and hurts still, and it never did before.
There is my rant. You can have your soapbox back now.
He recently went back to work after 6 months off, and had to wean off of the methadone, that was a scary couple of weeks. He is still in extreme constant pain, but we ran out of disability and time, and the jobs I can get would not pay for our bills.
What I have found out through all of this is that every doctor handles their prescription pads differently. We saw Drs that gave out pills like candy (although not the ones that worked for Hubby), and saw other Drs that would prescribe 3 days or a weeks worth of meds at a time, and we had to drive back and forth 60 miles each way to pick them up.
I really think they need a standard for pain management, both in the dental and medical fields. I do not want to find a Dr I really like, and than have issues about medications they will and won't prescribe. Right now, too much is left to personal opinion, or like with Lisa D, the dentist not knowing a drug or liking it.
Lisa -- I'm sorry if I offended you with my comments, that was not my intention. For me, it was one of those two sides to every story comments. If dentists are legally able to write CII in your state, I don't understand why he didn't work with you a little better and prescribe a CII he was familiar with. I totally agree that 4 days worth of pain medication was a very reasonable request.
I have been extremely fortunate with my personal doctor. His wife is a Pediatrician and she was my daughter's doctor from the moment she was born over 18 years ago. They are both very down to earth and realistic and not an arrogant bone in either of their bodies. I went into anaphylactic shock one summer due to an allergic reaction to a medication and was in intensive care overnight. The next time I saw him he was so apologetic that he couldn't come to see me. He had been notified that I was in the ER and he couldn't get away. Another time I had broken my ankle and went in through the ER and was being cared for by an Orthopedic Surgeon and my doctor was still there to check on me the next morning.
I hope your husband finds some relief for his back problems. Bring his bottles in to be counted? I'd blow a gasket if someone told me I had to do something like that! That is so out of line I can't believe they get away with doing it.

Ron. Demerol eh? I had a script of that for my migraines one time. It didn't work as well as the shots in the hospital when I'd be forced to go in for relief. I've never liked opiates, which is why I won't take them unless absolutely necessary and then, some of them I won't take at all because they plain do not work and make me throw up. Doesn't help if you can't keep the medication in you. Root Canal isn't fun when there is an infection present. And if they cap it off with a temporary filling, the pressure from the infection builds up and it feels like your tooth is going to explode. At that point though, I've wished it would just to relieve the pressure.
I talked to one of my best girlfriends today and she told me about her dentist and I'm going to call him after I have the surgery. She tells me he's pretty decent and cares about pain.

I'll check into the Lodine. Thank you for the info. I really appreciate it. I don't like opiates at all and if I can find something else that works, I'm all for it.
