Our daughter is 3 months old and has just started refusing the bottle. We have tried removing mommy from the room so she doesn't get distracted. She rarely breast feeds anyways. It doesn't matter if the bottle contains formula or breast milk. She is hungry but refuses to latch on to the bottle. She moves her head from side to side and starts to get fussy.

Sorry, no suggestions here. Lots of moms on here though. Hopefully someone has some advice for you.
I always think think formula must tast a lot like dried milk. I remember how NOT like milk that taste.
Our first child absolutely refused to take a bottle. He then stopped taking my wife's breast -- but only the left one. This had us very confused. I forget how we stumbled on the answer, but it turns out that he was getting too much at once and was choking on each mouthful. We solved the problem by reducing the amount of milk he got. For the breast, my wife would pump off a bit on the left breast to reduce the flow, and for the bottle, we found nipples with a much smaller aperture.
It make take some struggling to get her to try the bottle again, but if you just persevere hunger will overcome fussiness and then she'll realize that the problem has been solved (if that was the problem).
