A living will is one form of advance directive, leaving instructions for treatment.
If you were incapacitated what would you want.
I dont want to be kept alive by machines.
If i become unable to make choices i want the machine turned off, what about you??

If I am diagnosed with any type of cancer? I am refusing any kind of treatment other than pain medication, of which I plan to have enough to take all at once, to die as quickly as possable, when my condition deteriorates significantly.
If I am incapacitated, do not hook me up to any machines nor use any extrodinary, heroic measures to attempt to save me.

I don't think putting a feeding tube in would have helped him live longer, because of his liver problems, I think he would have died sooner.
He made me his medical surrogate because I was the one taking care of him and the one living near him.
However, I wouldn't want to be on machines for years. I would only want a chance, after that, let me go.
