The boy is wearing an elaborate and fanciful costume for fishing, and has landed a peculiar aquatic creature. I believe that the drawing is intended to represent a fish, but the creature could also be a small seal.
The "pier" resembles a very large bench extended over water at an unusual height.
At the end of the pier, two women are observing the unusual sea creature. One woman appears ready to curtsey before this phenomenon, while her companion attempts to steady her -or, perhaps, to push her into the water.

The card is typical of "art" cards (not based on photographs) circa 1910.
It is related to "comic cards", in that the illustration suggests a "story" and includes two women with expressions of surprise or admiration.

I am very fond of this card, and consider it to be a charming example of American Folk Art.
It makes a lovely screen-saver, too.

I believe that the child has captured a small seal and the curtsying woman is about to be pushed off the bench/dock.
