Just go to www.disneyparks.com and find the link to the right. Or go directly to http://disneyparks.disney.go.com/disneyparks/en_US/WhatWillYouCelebrate/index?name=FreeOnYourBirthdayPage to sign up. Choose which park you'd like admission to, and you're off!
I'm signing up my entire family. We had to give up our seasonal passes to the parks last Spring -- we just couldn't afford to renew them again. But if it's a child's birthday then we adults could just buy a 2-day resident pass and be able to use it again for the next child's birthday. But this special offer is good for everyone, not just children.
Now, if you are a Passholder already, this does not give you a special discount or reimbursement for that particular day. I do think if you mention to a castmember that it's your birthday, Disney parks are pretty good about celebrating with you. There is a special pin they'll give you to wear for this or other special occasions, such as an anniversary or a first visit to the park.
