It's nice to have certain friend who you can write out of the blue about anything:

D says: Maybe I should try to add that into the general section of our H&P. Instead of writing: Patient is pleasant lying down in bed, NAD (no acute distress), A/Ox4 (alert and oriented to person, place, time, situation)...I should write "Patient is whelmed."

D says: or just write it in chicken scratch on the actual note.
